The term "disability" refers to both mental and physical conditions which are permanent, ongoing, episodic, or of some persistence, and cause a significant limitation for the person in carrying out some of life's major activities. A disability may be visible, in the form of a physical limitation, or it may be an invisible disability, such as a learning disability.
Students who wish to receive accommodations from Accessibility Services will be asked to fill out a Self-Identification of Disability Form and submit it, along with appropriate medical documentation or the Verification of Disability Form, to the Coordinator of International and Accessibility Programs.
