Position Summary

As a member of the Hope Centre Health Care Inc., multi-disciplinary team, the Counselor will ensure the day to day operations of the Counseling Department.  This will be carried out under the direction of the Executive Director as directed by the Board policy and in keeping with Hope Centre Health Care Policies and Procedure and the Mission Statement.

Responsibilities

Client Care

1.  Facilitates the assessment, identification and analysis of service methods consistent with client's goal and objectives.
2. Actively engages the client in determining and developing a course of action which will include: identification of problem areas, establishment of treatment goals, facilitation of problem resolution and development of criteria for termination.
3. Establishes a counseling contract with clients; crisis counseling (2-3 weeks) short term counseling (under 6 weeks), long term counseling (6 weeks and longer), or group counseling and together with client works consistently towards treatment goals within an agreed upon time frame.
4. Implements the internal and external service delivery methods to address and meet the need of the client.
5. Utilizes case conference meetings to report and review specific case summaries as deemed appropriate.
6. Maintains client health records as per HCHC policy and procedures within the electronic medical record system.
7. Returns calls and answers letters promptly
8. Maintains counseling waiting list.
9. Initiates, develops, consults and maintains an ongoing awareness of community needs.
10. Responsible for provided complete case management for assigned clients.
11. Provides appropriate closure of counseling whenever possible

External Agency Contracts

1. Consults and collaborates with appropriate department/agency/community groups on a case by case basis.
2. Interprets the work of the department and HCHC to outside agencies.
3. Attends and/or initiates meetings outside the agency when necessary or requested by the Executive Director
4. Provided consultation and learning opportunities as requested.

Multi-Disciplinary Team Member

1. Functioning in a multi-disciplinary team setting, the Counselor will work with HCHC personnel in providing whole person health care to clients.
2. Participates and contributes in case conference, health planning conference meetings and other HCHC committee or task group when necessary.
3. Participates and contributes in staff meeting, staff in-services, department meetings and other HCHC Committee or task groups as necessary.
4. Maintains appropriate confidentiality as per HCHC policy
5.   Presents self and program in a positive and professional manner.
6. Uses appropriate channels of communication at both internal and external agency levels.

They will work in conjunction with other AIA leaders to ensure intentional care and development of staff (MPD, training), recruit new staff, and to build strategic partnerships with other ministries and churches.

Specifically, the Elite Initiatives Director will look to increase the influence and impact of chaplaincy within the elite athletic community in Canada. There are many leagues and other opportunities to expand chaplaincy and this leader will envision and work out a plan to step into these areas. Their mandate is also to continue, alter, or transition the current impact our chaplains have in various sport leagues and with national athletes.

Dorion Bible Camp is seeking a Camp Director to lead our year-round camp ministry in the Thunder Bay District of Northern Ontario. About the Role

local churches and community sports groups; the execution of camps and/or clubs programs and sports initiatives; provide the administrative support necessary to carry out an Action Club and Camp program. As an AIA staff, you will attend the bi-annual P2C Staff Conference and annual AIA Summit.
